Your Right to Receive a Good Faith Estimate
Under the No Surprises Act, uninsured and self-pay clients have the right to receive a Good Faith Estimate explaining the expected cost of health care services.
A Good Faith Estimate shows the expected charges for services that are reasonably expected for your care. It is based on information known at the time the estimate is created.
This page is a general notice of your rights. It is not an individualized Good Faith Estimate for your specific care.
Who Can Request an Estimate?
You have the right to receive a Good Faith Estimate if you are uninsured or if you choose not to use insurance for your care.
You may ask for a Good Faith Estimate before scheduling services. You may also receive one when services are scheduled, when required by law.
What the Estimate Includes
A Good Faith Estimate may include expected charges for services such as consultation, psychological services, telehealth services, evaluation services, or other scheduled care provided by the Practice.
The estimate will be based on the expected type, frequency, and length of services known at the time it is prepared.
The estimate is not a contract. Actual charges may differ if your needs change, if the type or frequency of services changes, or if additional services are requested or clinically appropriate.
When You Should Receive an Estimate
If you schedule services at least 3 business days in advance, you generally have the right to receive a Good Faith Estimate before the service. You may also request a Good Faith Estimate before scheduling care.
If you request an estimate before scheduling care, the Practice will provide it within the timeframe required by federal law.
If Your Bill Is Higher Than the Estimate
If you receive a bill that is at least $400 more than the Good Faith Estimate you received from this Practice, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.
You should save a copy or photo of your Good Faith Estimate and the bill.
